Printable Library Card Applications
If you prefer not to apply for a card online, or you are not a Greenwich Resident (CLICK HERE to see our requirements), you may print out an application form. Please bring it to the library with two pieces of appropriate identification to apply for your new card. Children under 12 years of age should use the "Children's Application." The signature of a parent or legal guardian is required.
ADULT APPLICATION
By applying for a Greenwich Library card, you agree to the following terms:
"I accept responsibility for all transactions made on
this card. This card remains the property of Greenwich Library, and its use is
subject to Library policy and Connecticard (http://www.connecticard.org/)
agreements. There is a replacement fee for lost cards. I will report lost and
stolen cards or change of address immediately."
APPLICATION FOR CHILDREN'S LIBRARY CARD
By applying for a Greenwich Library card, you agree to the following terms:

SOLICITUD PARA LA TARJETA DE LA BIBLIOTECA PARA ADULTOS
Para su conveniencia, si usted desea solicitar una tarjeta de
la biblioteca, usted puede imprimir y completar el formato de solicitud de
tarjeta y traerla junto con dos formas diferentes de identificaciòn. Niños menores de 12 años deben
usar el formato de solicitud para niños y uno de los padres o guardián legal debe firmar la solicitud.
Esta tarjeta es propiedad de la Biblioteca de Greenwich. Su uso esta sujeto a las
polìticas de la biblioteca y del Programa Cooperativo de las Bibliotecas
Pùblicas en Connecticut. Existe un costo por reemisiòn de esta tarjeta. Pèrdidas
de tarjeta y cambios de direcciòn deben ser reportados inmediatamente.

プリント可能な図書館カード申込書
グリニッチの住民であれば申込書をプリントし記入した上で、グリニッチ在住を証明する書類と運転免許書かパスポートを持って図書館にお越し下さい。また、１２歳未満のお子様は専用の申し込み用紙をプリントして記入した上で保護者と図書館にお越し下さい。
グリニッチ図書館カードの発行に伴いグリニッチ図書館の貸出し規則に従うことに合意します。このカードの所有権はグリニッチ図書館にあり、会員規約とConnecticard規約(http://www.connecticard.org/)を承認した上で、利用します。カードを紛失した場合は再発行に費用がかかります。カードを紛失した場合は直ちにグリニッチ図書館に連絡します。
